Home Visiting CoIIN

Florida MIECHV was actively involved in the Home Visiting Collaborative Improvement and Innovation Network (HV CoIIN) from May 2014 through August 2016. 

The HV CoIIN is the first national initiative using the Institute for Healthcare Improvement’s Breakthrough Series Model to close the gap and improve critical outcomes for families. 

Two Florida MIECHV programs, Northeast Florida Nurse Family Partnership and Pinellas Parents as Teachers participated for over two years, with teams from eleven other states, in the CoIIN CQI Learning Collaborative.  Each team used rapid testing for improvement methods, shared best practices and built their CQI capacity. 

Initially, Pinellas focused on improving developmental screening and surveillance and later changed their focus to family engagement and retention in Phase 2 of the Learning Collaborative.  The Northeast Florida team focused on breastfeeding in Phase 1 and Phase 2 and was able to increase the duration of breastfeeding in their participants.

Each topic area was guided by expert faculty and documents including charters, key driver diagrams and change ideas based on the latest evidence-based research and innovative thinking. 

Each month, the teams designed and completed Plan-Do-Study-Act (PDSA) cycles to test those changes and drive improvement.

State leads, including Florida’s Senior Manager, Allison Parish, taught portions of the learning sessions and shared best practices, lessons learned and CQI techniques.

The collaborative tracked individual agency and overall progress of the HV CoIIN using standardized outcomes and process measures for each target area. Each agency reported on these measures monthly as they tested and adapted the recommended changes.
